1. Once on the MYNOTE webpage, click the “Sign Up” button in the lower right corner.
2. After clicking the “Sign Up” button, fill in the information required. Make sure the social security number (or tax identification number) you have added to your MYNOTE account matches the number that was submitted to our office.
3. Once you have filled in all the required information, submit your account by clicking the “Sign Up” button. 4. You will then receive a link to your email asking you to create your password. If you do not see the link, check your junk mail.
5. Sign into your account with your new password and username.

If you have multiple accounts, they will all show up on your user account providing that the social security number or tax identification number matches the one in our system.
